† For children with NO INSURANCE, Medicaid or is Medicaid-eligible

† American Indian/Native Alaskan

† Under-insured (insurance does not pay for vaccines)

† No CHIP accepted

† Child Must be accompany by a parent or guardian

† Must bring child’s Immunization Record/Shot Record

† First come first served basis
