This year’s Summer Bash activities will include:

• Backpacks & School Supplies- while supplies last, school-age child must be present
• Immunizations 2 months- 18 years old
  • Must have original shot record- school health records not accepted
  • We provide immunizations to those who:
    • have Medicaid
    • have no insurance
    • have insurance that does not cover immunizations
• Variety of Health Screenings such as Eye Exams, Dental Exams
• GIVEAWAYS for all Attendees!
• Sports Physicals
• Face Painting
• Healthy Snacks Demonstration by a Health Nutrition Educator
• Community Resources

Please bring…

• Current check stubs
• Each child’s birth certificate and Social Security card
• State-issued ID (driver’s license, ID card, etc.)
