CHILDREN'S IMMUNIZATIONS: Aldine Family Hope Center

Your child is eligible if he/she is between 2 months to 18 years: 

† Has Medicaid or (If your child has Medicaid, please bring number and eligibility date)

† Has No Insurance or Under-insured (insurance does not pay for vaccines) or

† Is American Indian/Native American or Alaskan Native

† Child Must be accompany by a parent or guardian

† Must bring child’s Immunization Record/Shot Record

† CHIP not accepted

† First come first served basis

**There will be a $20.00 donation fee for college entry Meningitis vaccine if you are 19years and older only.**
