† Children MUST be accompanied by PARENT or GUARDIAN

† American Indian/Native Alaskan

† Under-insured (insurance does not pay for vaccines)

† CHIP (Children’s Health Insurance Program) NOT accepted

† Must bring PRINTED child’s Immunization Record/Shot Record
